Key Benefits & Why They Matter

  1. Additional Clinical Oversight
Why it Matters: Progressive diseases lead to increasingly complex medical needs. Having an additional team of medical professionals in addition to your primary care physician offers an extra layer of support to regularly assess symptoms and medications—with the goal of preventing complications before they arise.
  2. Proactive, Symptom-Focused Care to Enhance Comfort & Wellbeing
Why it Matters: The supportive care team works to help manage common symptoms such as chronic pain, agitation, medication sensitivity, and sleep disturbances. This proactive approach helps adjust care plans before symptoms become severe, supporting comfort and quality of life.
  3. 24/7 Access to Care & Enhanced Communication
Why it Matters: Medical needs don’t always happen during business hours. Silverado Supportive Care provides 24/7 triage support, meaning:
    • Immediate assessment of any change in condition
    • Timely medical orders or interventions
    • A supportive care clinician can be dispatched to the patient’s location—even in the middle of the night
    • This proactive approach has been shown to reduce unnecessary hospitalizations. Additionally, the supportive care team maintains open communication with the patient’s primary care physician to ensure their ongoing care needs are managed effectively.
  4. Coordination of Care Across Healthcare Providers
Why it Matters: The supportive care team focuses on preventing unnecessary hospitalizations through proactive care and symptom management. In the event you or your loved one requires hospitalization or specialized care, the supportive care team will coordinate communication between emergency departments, hospitalists and specialists. This enhanced support helps reduce hospital visits, medication complications, and stress for both the patient and their family.
  5. Enhancing the Care Experience with Specialized Support
Why it Matters: Silverado Supportive Care further enhances clinical support by providing timely medical orders for medications, equipment, or additional services when medically necessary. Patients and families can feel confident knowing that there is access to an additional layer of symptom management and personalized attention.
  6. Medicare Covers This Benefit
Why it Matters: Many don’t realize that Medicare covers supportive care services for individuals with serious illnesses. Simply having a diagnosis is qualification for this benefit. This means additional medical oversight with minimal out-of-pocket cost is available—an opportunity to ensure a higher level of care as needs evolve.

  • What is Supportive Care?

Supportive Care is a Medicare-approved benefit, also referred to as “Palliative Care.” It provides specialized medical care designed to improve the quality of life for individuals with serious illnesses, such as dementia. This care focuses on symptom management, comfort, and coordination while a person continues receiving treatment for their condition throughout the progression of the disease. For benefit purposes, we use the term supportive care to refer to palliative care under Medicare’s definition.

  • How do I enroll my loved one in Silverado Supportive Care service?

  • Can supportive care help prevent my loved one from being hospitalized?

Yes. Many medical concerns can be managed on-site without the disruption of a hospital visit. Silverado Supportive Care’s palliative care team monitors symptoms, provides urgent medical attention, and coordinates with the primary care physician to address health concerns before they escalate. This proactive approach reduces unnecessary hospitalizations while ensuring your loved one receives the right care in a familiar environment.

  • What if my loved one needs to go to the hospital? How does supportive care help?

The supportive care team’s goal is to prevent unnecessary hospitalizations. However, if your loved one requires hospitalization, the supportive care team will assist in coordinating communication between hospital staff, specialists, Silverado Memory Care nurses, and your family. This support helps streamline care, reduce hospital readmissions, and ensure treatment aligns with your loved one’s needs and goals.
